Well if you have downloaded the POI you will want to associate a bmp file with the POI. Just save the bmp file the same as the POI file. The file names must be the same. The only difference will be the extensions.
Keep the names identical to the Extension. Make sure your Icon is in BMP format. The name is CASE Sensitive for the GPS so I usually use cut and paste and then change the extension to .BMP
I found this on the Garmin site. It seems that somehow POI loader creates and incorporates the icon bmp file into the POI file. Does anyone know if this really works? I'm wanting to see the custom icons on an Oregon 600.

Custom icons can be used with POIs on a device. The icons display on the map to mark a POIs location. The POI Loader integrates the .bmp image file into the .gpi while loading. Custom image files must be in the same directory as the .csv or .gpx files for POI Loader to process them correctly. The image must also be in the proper format, size, and color palette:
Format .bmp file type
8 or 16 bit RGB color palette
24x24 pixels in size
Transparency color is magenta RGB: R=255, G=0, B=255
The name must be identical to the .csv or .gpx file
So far I have switched to a PC from my MAC, downloaded bit map file and CSV files, checked that the names are identical with the exception of the extensions. I have put these files in the same folder and uploaded with Garmin POI Loader.
I do get the POIs loaded fine but STILL THE ICONS ARE DEFAULT GARMIN DOTS, NOT THE BIT MAP FILE ICONS I DOWNLOADED.
